Physical Therapist Home Care
Job Description:
Evaluates the home health physical therapy patient
Establishes and implements a treatment program based upon goals and in accordance with physician orders
Communicates and coordinates with other home care services
Provides documentation according to agency policy
Reports to Director of Patient Services
Job Requirements
Bachelor's degree required
Graduate of a Physical Therapy curriculum accredited by the APTA
Current Physical Therapist license in the state of New York
Must have at least 2 least years' experience in Physical Therapy.
Current BLS Health Care Provider Certification
Demonstrates effective communication skills
Demonstrates initiative and is able to function independently without direct supervision and as a member of a team
Must be flexible to meet patient care needs
Demonstrates the ability to initiate, accept, and adapt to change
Demonstrates the ability to drive to patient homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs
Coordinates services with other disciplines and community agencies to meet patient needs
Demonstrates ability to work in patient homes in less than ideal states of cleanliness
Demonstrates good organizational and time management skills to meet patient care needs and agency requirements
Must have current auto insurance at NY State minimum required levels
Must possess and maintain current NY driver's license
When appropriate and feasible, demonstrates and teaches physical therapy techniques, exercises and appliance/equipment use to patient/family and appropriate team members.
