Continuing Care Coordinator Psychiatry/Addiction Medicine

Under general supervision of the director of outpatient Psychiatry, the Continuing Care Coordinator will perform non-professional aspects of the care coordination, discharge planning and transfer process. The Continuing Care Coordinator completes and executes the care and transition plan for the patient with the guidance, and arranges post discharge services and facility transfers. The role will provide a prompt, systematic, reliable, and efficient supportive presence to the physicians and nursing staff in their continuing care activities in order to enhance the delivery of these services to patients. In addition, this position requires special skills that include knowledge of government programs, area facilities and insurance requirements for behavioral health. Staff must maintain ongoing communication with Case Management and professional staff, be able to take direction and to assess data reflective of the patient's status and their financial needs and interpret the appropriate information to assist with the continuing care arrangements. Meet with patients and families and community organizations as appropriate. Facilitates transfers, such as residential psychiatric programs or abuse rehab facilities. Assists in filling out required paperwork for transfer or outpatient discharge services, such as SPOA applications. Including arrangement for transportation, insurance pre=authorizations, and any other services needed to facilitate care transitions to and from the Bassett Healthcare Network. Transmits clinical information to payers and approval. Maintains metrics related to care transitions and readmissions. Works with primary care clinic staff and psychiatry staff to facilitate referrals for patients in need of additional services. Ensures access for patients through appointment scheduling, identifying openings for patient visits and collaborating with leadership to avoid no show/missed appointments. Develops networks and relationships with various referral sources in the psychiatric and substance communities in the multi-county region covered by the Bassett Network. Establish respectful working relationships with referral agencies outside of the Bassett Network to create and system of resources for behavioral health patients in need of inpatient, residential or outpatient substance abuse treatment and facilitate referrals from those agencies to Bassett providers for services where appropriate.
Minimum Requirements:
Associates degree required (preferred: Human Services, Chemical Dependency, Social Work or Psychology)
High school diploma or equivalent may be considered with an additional 2 years of experience.
2 years of experience working with behavioral health patients in rehab or social services setting
Recent graduates considered with relevant coursework, internship or volunteer history
Basic computer skills and previous health-related experience preferred.
Ability to function as a self starter and an active team member .
Demonstrates competency in required skills relevant to area of practice.
Customer service, basic computer and telephone skills required.
Ability to establish and maintain respectful relationships with referral agencies outside of the Bassett Network.
Physical Requirements:
Requires long periods of concentration and ability to multi-task.
Requires ability both to be mobile and to sit for long periods of time.
Travel to Regional Health Centers 1 or more times per week.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
